The purpose of this case report was to show the role of keratinized gingiva around dental implants. Some studies represented that appropriate width and thickness of keratinized gingiva which is adjacent to implant play a important role in implant successes. Other studies on the relationship between implants and keratinized gingiva proved the results that supportive postimplant therapy was more important than keratinized tissue augmentation around implants. In this case, #11, #21 implants fixture of 33-year-old male patient has exposed and had trouble in maintaining oral hygiene around implants. After several times of apically repositional flap and connective tissue graft, sufficient amount of keratinized gingiva could be adjacent to #21, 11 implants. There was no significant change in gingiva and bone level for three years.
